Medical Records Requests

Following the closure of Peace By Piece Counseling, all clinical records will continue to be securely maintained in accordance with HIPAA and applicable record-retention requirements.
To help protect your privacy and ensure continuity of care, we strongly encourage records to be transferred directly to your new healthcare provider through a secure, HIPAA-compliant method, such as fax or secure electronic transmission.

Whenever possible, we kindly ask that you provide your new provider's contact information and secure fax number with your request.

While direct provider-to-provider transfers are preferred, clients may also request copies of their own records, which will be provided in accordance with applicable privacy laws.

How to Request Your Medical Records

To request your medical records from Peace By Piece Counseling, please complete the Release of Information form through our secure TherapyPortal.

Step 1: Access the Client Portal

Step 2: Sign In

Step 3: Locate the Release of Information Form

Once logged in, navigate to your available documents or forms and select Release of Information.

Step 4: Complete the Form

Please provide all applicable information, including:

Your full legal name and date of birth.
Your contact information.
The specific records you are requesting.
The name of the healthcare provider or practice receiving your records, if applicable.
The receiving provider's secure fax number or other secure delivery information.
Your signature and the date of your request.

Step 5: Submit Your Request
